Dementia Coordinator for Tenterden

Emma Reader is our Dementia Coordinator for Tenterden, Kent and is based here at Tenterden Social Hub and Dementia Centre. Covering Ashford Primary Care Network, Emma is a single point of contact for advice and support for dementia across the Tenterden and rural areas and offers a full care pathway from diagnosis to end-of-life care.

Our Dementia & Me Coordinator is a free support service and will visit you in your own home and develop a personalised support plan for you as well as offering a wide range of support and advice.

How can Emma help?

Dementia & Me Coordinators help with:

  • Managing changing needs throughout the dementia journey.
  • The importance of a diagnosis and attendance at Memory Assessment appointments with you and your carer.
  • Identifying and arranging access to appropriate support groups for your diagnosis and accompanying you take the first steps.
  • Offering emotional support and advice around diagnosis and difficulties arising from dementia symptoms and respite care.
  • Understanding the available services locally and signposting to other organisations with specialist expertise in dementia support or for other support needs.
